Follow this procedure if you are injured on the field

If you are injured in a scholastic game which is high school or junior high only, your employer is the WIAA. Tell this to your doctor or ER admitting office and they will complete the appropriate form and they will send it to WIAA, your employer. For all other ball, college, Koufax, CYO, summer, PSSBL et al, your employer is NBUA. The doctor's office will send the form to NBUA at Ken Sack, Treasurer's office of 14322 32nd NE Seattle 98125. He'll validate the form and mail it off to L & I for further processing. This is all you have to do. Just remember who your employer is the key. WIAA is 435 Main Ave. South, Renton 98055.
